Rotator Cuff Arthropathy

Rotator cuff arthropathy is a form of shoulder arthritis that develops after a large, long-standing rotator cuff tear. The rotator cuff is a group of four muscles and tendons that keep the shoulder stable and allow you to lift and rotate your arm. When these tendons are severely torn and cannot be repaired, the shoulder loses its normal mechanics.

Without a functioning rotator cuff, the ball of the shoulder joint gradually moves upward instead of staying centered in the socket. Over time, this abnormal movement causes the cartilage to wear away, leading to arthritis, pain, weakness, and loss of shoulder function. Rotator cuff arthropathy most commonly affects adults over the age of 60, but it can occur at any age following a chronic, irreparable rotator cuff tear.

Causes of Rotator Cuff Arthropathy

Rotator cuff arthropathy develops gradually over many years. Common causes include:

  • A large rotator cuff tear that has been present for a long time
  • An untreated or irreparable rotator cuff tear
  • Repeated wear and tear of the shoulder with aging
  • Previous shoulder injuries or trauma
  • Failed previous rotator cuff surgery
  • Less commonly, inflammatory arthritis or nerve injuries that weaken the rotator cuff muscles

Not everyone with a rotator cuff tear develops arthropathy, but the risk increases when the tear is large and cannot be repaired.

Symptoms of Rotator Cuff Arthropathy

Symptoms typically develop slowly and worsen over time. Common symptoms include:

  • Deep, aching pain in the shoulder
  • Pain that worsens with reaching or lifting
  • Difficulty raising the arm overhead
  • Significant weakness of the shoulder
  • Loss of shoulder motion
  • Grinding, popping, or catching sensations with movement
  • Pain that interferes with sleep, especially when lying on the affected shoulder
  • Difficulty performing everyday activities such as reaching into cabinets, getting dressed, or washing your hair

Many patients notice that while pain is bothersome, the greatest limitation is often the inability to lift the arm.

Treatment Options

Treatment depends on your symptoms, activity level, overall health, and the severity of arthritis and rotator cuff damage.

Non-Operative Treatment

Many patients experience significant improvement without surgery. Non-surgical treatment may include:

  • Activity modification: Avoiding activities that worsen pain while maintaining gentle shoulder movement.
  • Physical therapy: Exercises to improve flexibility, strengthen the remaining shoulder muscles, and maximize function.
  • Anti-inflammatory medications: Over-the-counter or prescription medications may help reduce pain and inflammation.
  • Ice or heat therapy: These can help manage discomfort and stiffness.
  • Corticosteroid injections: A cortisone injection into the shoulder joint may provide temporary pain relief and improve function.
  • Biologic injections: In selected patients, treatments such as platelet-rich plasma (PRP) may be discussed, although current evidence has not consistently shown benefit for advanced rotator cuff arthropathy.

Although these treatments can reduce pain and improve function, they do not reverse the arthritis or repair the torn rotator cuff.

Operative Treatment

Surgery may be recommended when pain becomes severe, shoulder function continues to decline, or non-surgical treatments no longer provide relief.

Reverse Total Shoulder Replacement

The most effective and commonly performed surgery for rotator cuff arthropathy is a reverse total shoulder replacement.

Unlike a standard shoulder replacement, a reverse shoulder replacement changes the position of the ball and socket. This design allows the deltoid muscle to lift the arm instead of relying on the damaged rotator cuff.

  • Increased shoulder strength and function
  • Reliable long-term outcomes for most patients
  • High patient satisfaction

For many patients with advanced rotator cuff arthropathy, reverse shoulder replacement offers the best opportunity to return to everyday activities with significantly less pain.

Other Surgical Options

In select situations, other procedures may be considered, including:

  • Arthroscopic debridement: Cleaning inflamed tissue and smoothing rough surfaces to decrease pain in patients who are not candidates for shoulder replacement.
  • Partial rotator cuff repair: Occasionally performed when some remaining tendon can be repaired to improve function.
  • Tendon or muscle transfers: May be appropriate for younger, active patients without significant arthritis who have irreparable rotator cuff tears.
  • Shoulder fusion (rare): Reserved for very unusual circumstances when other reconstructive procedures are not possible.

Outlook

Rotator cuff arthropathy is a progressive condition, but effective treatments are available. Many patients achieve excellent pain relief with conservative care, while those with more advanced disease often experience dramatic improvements in comfort, mobility, and quality of life after reverse total shoulder replacement.
