Non-Surgical Treatment for Shoulder Pain

Expert Care Without Surgery

Not every shoulder problem requires surgery. In fact, many patients experience significant pain relief and improved function with the right combination of non-surgical treatments.

At Midwest Orthopedic & Musculoskeletal Alliance (MOMA), our goal is to identify the source of your shoulder pain and develop a personalized treatment plan that helps you recover as quickly and safely as possible. Our fellowship-trained orthopedic specialists, primary care sports medicine physicians, physical medicine and rehabilitation physicians, interventional pain specialists, therapists, and rehabilitation team work together to provide comprehensive care tailored to your needs.

Whether your shoulder pain is caused by arthritis, rotator cuff disease, bursitis, tendonitis, frozen shoulder, instability, or a sports injury, we offer a full range of evidence-based non-operative treatment options.

Conditions Commonly Treated Without Surgery

Many shoulder conditions improve without an operation, including:

  • Rotator cuff tendonitis and many partial rotator cuff tears
  • Shoulder impingement syndrome
  • Bursitis
  • Frozen shoulder (adhesive capsulitis)
  • Biceps tendonitis
  • Arthritis of the shoulder and AC joint
  • Shoulder sprains and strains
  • Overuse injuries
  • Throwing injuries in athletes
  • Mild shoulder instability
  • Many shoulder fractures after appropriate evaluation
  • Recovery following shoulder dislocations

Even when surgery is eventually recommended, non-operative treatment often helps reduce pain, restore motion, and prepare the shoulder for a better recovery after surgery.

Comprehensive Evaluation

Successful treatment begins with an accurate diagnosis.

Your evaluation includes:

  • A detailed discussion of your symptoms and medical history
  • A comprehensive shoulder examination
  • Assessment of strength, flexibility, stability, and range of motion
  • Evaluation of your work, recreational activities, and athletic goals

When appropriate, advanced imaging may include:

  • Digital X-rays
  • Ultrasound
  • MRI
  • CT scans

Our specialists carefully evaluate all of the structures around the shoulder to determine the true source of your pain before recommending treatment.

Treatments

Physical Therapy

Physical therapy is one of the most effective treatments for many shoulder conditions.

Through our extensive network of experienced orthopedic physical therapists, we develop individualized rehabilitation programs designed to:

  • Reduce pain
  • Restore flexibility
  • Improve range of motion
  • Strengthen the rotator cuff and shoulder blade muscles
  • Improve posture and shoulder mechanics
  • Restore normal movement patterns
  • Improve balance and coordination
  • Safely return you to work, sports, and everyday activities

Our therapists communicate closely with your physician to ensure your rehabilitation progresses appropriately and is adjusted as your shoulder improves.

Activity Modification

Sometimes small changes in daily activities can significantly reduce shoulder pain while allowing injured tissues to heal.

Your physician may recommend:

  • Temporarily avoiding painful overhead activities
  • Modifying work or sports techniques
  • Adjusting lifting mechanics
  • Gradually returning to activity as symptoms improve

Our goal is to keep you active while protecting your shoulder from further injury.

Medications

Several medications may help reduce pain and inflammation, including:

  • Acetaminophen (Tylenol®)
  • Non-steroidal anti-inflammatory medications (NSAIDs), such as ibuprofen or naproxen
  • Prescription anti-inflammatory medications when appropriate
  • Topical anti-inflammatory creams or gels

Your physician will recommend the safest option based on your overall health and medical history.

Corticosteroid Injections

Corticosteroid injections can provide significant relief for many painful shoulder conditions by reducing inflammation within the joint or surrounding tissues.

These injections may be helpful for:

  • Shoulder arthritis
  • Bursitis
  • Rotator cuff inflammation
  • Frozen shoulder
  • AC joint arthritis
  • Biceps tendon inflammation

Many injections are performed using ultrasound guidance to improve accuracy and ensure medication is delivered to the intended location.

Although injections can provide excellent symptom relief, they are only one part of a comprehensive treatment plan and are often combined with physical therapy.

Orthobiologic Treatments

For selected patients, regenerative medicine techniques may be considered as part of a comprehensive treatment plan.

Platelet-Rich Plasma (PRP)

PRP is created from a small sample of your own blood. After processing, the concentrated platelets are injected into the injured tissue to deliver growth factors that may support the body’s natural healing response.

PRP may be considered for selected patients with:

  • Partial rotator cuff tears
  • Tendon injuries
  • Chronic tendonitis
  • Early arthritis
  • Certain sports injuries

Not every patient is a candidate, and your physician will discuss whether PRP is appropriate based on your specific condition and the latest scientific evidence.

Image-Guided Injections

For some shoulder conditions, image-guided injections improve both diagnostic accuracy and treatment effectiveness.

Using ultrasound guidance, physicians can precisely place medication into:

  • The shoulder joint
  • The subacromial bursa
  • The AC joint
  • The biceps tendon sheath
  • Other specific areas of inflammation

This precision often improves both safety and treatment outcomes.

OrthoLazer® Photobiomodulation Therapy

MOMA offers OrthoLazer® high-powered laser therapy, also known as photobiomodulation therapy, as an important part of our comprehensive non-surgical treatment program.

This painless, non-invasive treatment uses specific wavelengths of light to stimulate the body’s natural healing processes.

Potential benefits include:

  • Reduced pain
  • Decreased inflammation
  • Improved circulation
  • Reduced muscle spasm
  • Improved flexibility and mobility
  • Reduced joint stiffness
  • Enhanced tissue healing
  • Decreased reliance on pain medications

Laser therapy is frequently combined with physical therapy and other conservative treatments to maximize recovery.

Our OrthoLazer centers in Franklin and Brookfield provide convenient access to this advanced technology.

Lifestyle and Home Exercise Programs

Your recovery continues outside the clinic.

Our team provides personalized home exercise programs that help:

  • Maintain flexibility
  • Improve strength
  • Prevent stiffness
  • Reduce the risk of future injury
  • Maintain long-term shoulder health

We also discuss posture, workplace ergonomics, sports mechanics, and activity modifications that can help protect your shoulder.

When Is Surgery Recommended?

Although many shoulder conditions improve without surgery, some injuries require an operation to restore function or relieve persistent pain.

Surgery may be recommended when:

  • Pain continues despite appropriate non-surgical treatment
  • Significant weakness persists
  • A complete tendon tear is unlikely to heal without surgery
  • Arthritis severely limits daily activities
  • Shoulder instability continues despite rehabilitation
  • A fracture requires surgical stabilization

If surgery becomes the best option, your physician will discuss all available procedures and develop an individualized treatment plan.
