As recently as 20 years ago, it was difficult to diagnose and effectively provide labral tear shoulder treatment. But advancements in arthroscopy are now allowing doctors to use miniature cameras to see labral damage and repair it surgically.

Anatomy and Causes of a Labral Tear
The shoulder is the most mobile joint in the body. It is made up of the ball (the head of the upper arm bone) and the socket (the glenoid) of the shoulder blade. Around the edge of the socket is a ring of strong cartilage called the labrum. The labrum deepens the socket, helps stabilize the shoulder, and serves as an attachment point for important ligaments and the biceps tendon.
A labral tear occurs when this cartilage is damaged or pulled away from the socket. Labral tears can happen in several ways, including:
- A fall onto an outstretched arm
- A direct blow to the shoulder
- A shoulder dislocation or partial dislocation
- Repetitive overhead activities such as throwing, swimming, or weightlifting
- Normal wear and tear with aging
- Heavy lifting or sudden pulling injuries
There are several different types of labral tears. Some occur in the front or back of the shoulder following instability or dislocations, while others involve the top of the labrum where the biceps tendon attaches (commonly called a SLAP tear).
Labral Tear Symptoms
Symptoms vary depending on the size and location of the tear but commonly include:
- Deep aching pain inside the shoulder
- Pain with reaching overhead or behind your back
- Clicking, popping, catching, or grinding sensations
- A feeling that the shoulder may “slip,” “give out,” or become unstable
- Weakness during lifting or throwing
- Decreased range of motion
- Pain during sports, exercise, or repetitive activities
- Difficulty sleeping on the affected shoulder
Some patients experience shoulder instability after a traumatic injury, while others mainly notice pain during activity.
Diagnosing a Labral Tear
Diagnosing a labral tear begins with a detailed medical history and a thorough physical examination by one of our orthopedic shoulder specialists.
During your evaluation, your physician will:
- Discuss how the injury occurred
- Review your symptoms and activity level
- Evaluate shoulder motion, strength, and stability
- Perform specialized tests that can identify different types of labral injuries
Imaging studies may include:
- X-rays to rule out fractures, arthritis, or other bone problems
- MRI to evaluate the soft tissues of the shoulder
- MR Arthrogram (MRI performed after contrast is injected into the shoulder joint), which is often the most accurate test for detecting many labral tears
Because other shoulder problems such as rotator cuff tears, instability, arthritis, or biceps tendon injuries can cause similar symptoms, an accurate diagnosis is important to determine the best treatment plan.
Labral Tear Shoulder Treatment Options
Non-Surgical Treatment
Many patients improve without surgery. Treatment depends on your age, activity level, type of labral tear, degree of instability, and your personal goals.
Non-surgical treatment may include:
- Activity modification to avoid painful movements
- Rest from sports or repetitive overhead activities
- Anti-inflammatory medications when appropriate
- Ice to reduce pain and inflammation
- Physical therapy to restore motion, improve strength, and stabilize the shoulder
- Corticosteroid or orthobiologic (PRP) injections in selected patients to reduce inflammation and improve comfort
Many patients with degenerative or small labral tears achieve excellent results with conservative treatment.
Surgery may be recommended if:
- Pain persists despite non-surgical treatment
- The shoulder continues to dislocate or feel unstable
- The tear is causing mechanical symptoms such as catching or locking
- You are a competitive athlete or perform repetitive overhead work
- The tear occurred after a significant injury and is unlikely to heal without repair
Surgery
When surgery is needed, it is most commonly performed using minimally invasive arthroscopic (keyhole) techniques. Through several small incisions, a tiny camera and specialized instruments are inserted into the shoulder, allowing your surgeon to examine the entire joint and treat the injured tissues while minimizing damage to surrounding structures.
Depending on the type of labral tear, surgery may include:
- Arthroscopic labral repair using small suture anchors to securely reattach the torn labrum to the socket
- SLAP repair for selected tears involving the upper labrum
- Biceps tenodesis, which may be recommended instead of SLAP repair in many adults over age 35–40 or in patients with degenerative changes involving the biceps tendon
- Capsular tightening with labral repair (capsulolabral repair) for patients with recurrent shoulder instability
- Arthroscopic debridement, in which frayed or damaged tissue is trimmed when repair is not necessary or would not improve outcomes
Your orthopedic surgeon will recommend the procedure that offers the best chance for long-term pain relief, stability, and return to activity based on your individual injury.
Recovery After Shoulder Labral Repair Surgery
Recovery following labral repair occurs gradually as the repaired tissue heals. Although each patient is different, recovery generally includes:
First 4–6 weeks
- Your arm is protected in a sling.
- Gentle motion exercises begin under the guidance of a physical therapist.
- Pain and swelling gradually improve.
6–12 weeks
- Range of motion continues to improve.
- Strengthening exercises are gradually introduced.
- Most daily activities become easier.
3–6 months
- Strength, endurance, and shoulder function continue to improve.
- Many patients return to work, recreational activities, and non-contact sports.
4–9 months
- Athletes and patients participating in overhead or contact sports typically complete a structured return-to-sport program before returning to full competition.
Physical therapy is one of the most important parts of recovery and plays a critical role in restoring shoulder motion, strength, stability, and confidence.
